coil pack went boom

So my buddies 97 T4R SR5 WITH 270K blew the coilpack to pieces when he was driving, so I pulled the. Coilpack out of the cylinder and the plug came with it, it looks as if there was a small fire as well. We are thinking the threads in the head gave up the ghost and shot the plug into the coilpack. Any one have any thoughts or any ideas?

Side note, kind of new on this forum thing so bare with me.

It looks to me like there was a time-sert, thread repair done in that plug hole already. See the nubs near the spark plug base? They are what set the insert into the new threads made by the special thread tap.

You're most likely gonna need a new head to get it right, or a damn good welder.

I noticed that also,and the after market bolt on just that particular coil that failed gives indication that there was a prior work done where the factory bolt was missing at some point .

Looks like the Timesert is still threaded on the plug which means it was a prior repair that has let go. As was said, time for a head repair (unlikely) or replacement.
 
We were leaning towards replacement, probably just going to find some used ones unless someone knows a good but cheap pave to get new ones

You know come to think of it, that make a lot of sense why it failed I didn't even factor in that I may have already had that timesert already in it. When I owned it several miles ago that cylinder had a newer coil pack and a single prong plug in it. And I never thought that had already a timesert in it. I just thought someone didn't know what there where doing. Thanks guys!
