P0340 camshaft pos sensor mystery

Hello all,
I've done a lot of searching and reading, I need help.
1996 4runner, 3.4, 251,000k

Cel code P0340 (camshaft pos sensor) keeps coming on, above 2500 steady rpms for more than a few seconds.
I reset the code, can drive forever as long as I don't go above 2500 for more than 5 or 10 seconds.

Yesterday, I changed the t-belt, tensioner, cam sensor, crank sensor, ohmed wires, checked voltage at sensors and computer.

I know the wiring, I know how the sensors work, I've checked the timing marks ten times. For fun, I disconnected (1 at a time) MAF, O2 sensors, and ran the engine cel came on again exactly the same.

Please help

@210k miles 2years ago. I replaced head gaskets, coils, plugs, waterpump, t-belt, tensioner, idler pullys.

How about the "other side" of the cam sensor. Chipped tooth maybe ?

I looked at each sprocket/gear closely, I don't see anything wrong.

If it were mine and I was certain of the circuit integrity(wiring, dirt, etc.), I would replace the sensor itself. It.s really the only "active" part aside from the ecu.

So I took it to a mechanic today. After a few hours he called and said 1500$ plus labor for a new wiring harness, because some of the sheilding is missing. I said, no thanks, leave it, I'll pick it up tomorrow.
