It really depends on how much rake you want. You don't "need" anything in the rear even with 5100's at the 1.75" setting, but there with anything loaded in the trunk you'll be reverse raked. At .85 you'll still have about 1" of factory rake, adding the OME Spacer will take you to about 1.5" factory rake (the factory rake is approx 2" give or take who you ask). If you are hauling stuff a lot the spacers are probably a good idea, if you only have light cargo or are riding empty most of the time then no need.

Alright guys. Here's an update that might help this thread.
This is after installing 5100's all around with the fronts at 1.75. Some pics make it hard to tell, whereas others make the rake more noticeable but my truck is INDEED not level. Perhaps it's because I have the heavy 3rd row seat option, I don't know.
Here is the setup as described above. I actually have a true noticeable drivers side lean as evidenced by my measurements (wheel center cap to fender). The driver rear measures 0.25 inches lower than the passenger rear height.
MY PLAN - is now to get these super affordable rear coils (OME 895e) AND a 10mm drivers side (ONLY) OME trim packer to correct the lean. This is the perfect option for me because if I get 1-2inches of rear lift - I should have a really nice overall lift with some factory rake *look to it AND I would have some stiffer and taller rear coils to compensate for when I have my heavy 1up hitch rack and bike(s) on there.
Here is current setup... Just have to get the wife's permission to buy the OME coils and 10mm Trim packer ($185 total)

Thanks for the update Tyler. I will be getting my 5100's hopefully for Fathers Day and I am going .85 for sure. I will be using your guy for the install very soon, so excited.
I have not noticed any drivers side lean like my old Tacoma. Maybe I should do some measuring before the install to see if I need a little spacer.

For sure, man! Yeah I would do some measuring, my guess is that with the very slight 0.85 lift you won't notice any lean at all... But take some measurements and maybe buy the trim packer anyway so you have it if you need it at the time of install. It's only $15! And although 10mm doesn't even seem thick enough to justify installing, it's pretty darn close to 0.25inches - which is how much lower my L rear side is. Haha

Looks great!! What rubber do you have on there?? I love the grey TRDP wheels on a classic silver truck!!

Are you close to rubbing with the offset wheels and the 265s? I think I'd like to go a bit bigger, but if if they are close to rubbing the ko2s even with the 265 look big!

Billies @.850 up front oem rear. That roof top carrier usually has a couple hundred lbs in it but I consider the lake a fairly level surface.

First off, thanks to all for posting their pics and experiences with the Bilsteins. It's really difficult deciding on what's the best setting. For those still searching, this is what I've concluded:
Stock springs, 0.85 setting up front, zero spacers or modifications rear
1. The 0.85 setting will level the truck by appearance of the frame. The wheel well gaps are different front and rear. This means you have to choose if you would like the frame leveled (I recommend this with the 0.85 setting) or the wheel wells even spaced (1.75), which will result in rearward rake made worse by adding passengers or weight to the rear.
2. Even at the 0.85 setting, the lift is noticeable over the stock sag and the ride is superb. While there are times that I wish the front was a touch higher, the frame and my nerf bars would be tilting rearward and would look ridiculous. This means that like many more knowledgeable than me have recommended, the 0.85 setting on stock coils is perfect.
Enough rambling...here is some pic porn.

Front is at .85, rear suspension is completely stock. Tires are stock-sized KO2s.
Here are some of my minor issues:
1. I noticed an issue with visibility. Specifically, my 5'3" wife noticed it. It is harder to see what is directly to the front-right of the vehicle, such as when pulling into driveways or parking.
2. When loaded, now the rear end looks like it sags more, so I need to improve that.
3. Wheel rub during tight turning, but I think that is mostly due to my spacers.
